OBJECTIVE

The Case Management Supervisor will be responsible for providing direction and guidance to Case Managers. This individual will coordinate staff with funding agencies to help clients achieve their individual goals while implementing rehabilitation programs for assigned consumers to achieve their personal goals.

REQUIRMENTS

Bachelors Degree in Psychology, Sociology, Vocational Rehabilitation, Counseling and Guidance or related degree. Equitable experience will be considered in lieu of degree. Applicable field or equivalent combination of training and experience with a minimum of one year in social services, counseling or rehabilitation setting, providing direct services to people with physical, emotional and/or developmental disabilities. A valid Colorado driver’s license without a history of violations is also required.

OTHER REQUIREMENTS/PREFERENCES

  • It is preferred that the incumbent have excellent written and verbal communication skills, write clear, concise and accurate reports and plans, and submit them in a timely fashion.

  • Must demonstrate the ability to interact with various types of internal and external customers, relate effectively to disabled persons, their parents and guardians and be empathetic toward them.

  • Must have a working knowledge of community resources, demonstrate initiative, work with a degree of independence and possess excellent time management skills.

  • Must have the ability to work independently, cooperatively and to exercise effective planning.

  • Must also conduct self in a professional manner and maintain a professional appearance that enhances the firm’s reputation.

  • Must have experience speaking effectively to various small groups in a presenting/informative role.

  • Must be able to program, train and be creative in motivating consumers toward individual success.

KEY RESPONSIBILITY AREAS {Essential Functions}

Case Management

The Case Management Supervisor will provide direction, supervision and case management for all assigned consumers to include: completing an initial rehabilitation plan and assessment, develop and implement an individual program plan and document progress. The incumbent will function as part of the interdisciplinary team and develop the individual rehabilitation program. The Case Management Supervisor will encourage independence by assisting consumers in problem solving by using accepted and innovative rehabilitation techniques and maintain a proactive environment.

Staff Support and Development

The Case Management Supervisor is responsible for all personnel issues for direct reports including monitoring performance and completing performance evaluations. As a team lead, the incumbent will supervise direct reports and ensure all case management staff are present for work and accountable for. The Case Management Supervisor will monitor the work of each staff member for accuracy and appropriateness while ensuring that all case management paperwork for CARF, DDD and funding agencies are turned in on appropriate due dates. The Case Management Supervisor will also assist case managers with problems and conflicts concerning funding agencies, parents, guardians and consumers. The incumbent will cover for staff when absent, attend intakes and function as the in-charge person during program coordinator’s or classroom lead’s absence.

Client/Program Tracking and Reporting

The Case Management Supervisor will maintain complete and accurate records and statistical data for each case as required. In addition, the Case Management Supervisor must complete all necessary documentation, e.g., incident reports, program plans, charting, quarterly and final reports. The incumbent will assist with filling out incident reports and reporting them to the funding source and state if necessary.

Relationship Management

The Case Management Supervisor will communicate and coordinate with other involved agencies and support personnel. In addition, the Case Management Supervisor will demonstrate good customer service with all clients. The Case Management Supervisor will deal effectively and calmly with non-routine situations following accepted guidelines and perform other duties as required by the Coordinator. The incumbent will attend all Rehabilitation meetings and relay pertinent information to the rest of the team. In addition, the Case Management Supervisor will also attend training and sign staff up for necessary trainings.

Research and Industry Involvement

The Case Management Supervisor is required to research, keep updated by reading trade journals as needed to be aware of trends, theories or initiatives within Case Management and other applicable within the industry. The incumbent will seek out seminars and symposiums for ideas and stay current with trends in new software, tools and resources available. The incumbent will also maintain a good working knowledge of Goodwill Industries’ policies and procedures as well as CARF and DDD rules.
